Champion Toolkit


Champions aiming to inspire others may wish to use the resources in the toolkit. Champions may include physician or medical home leaders, health or social service planners or health or community advocate or partners such as FCSS, municipality, or social agency. What questions might you ask, what key messages do you weave into your conversations and meetings and how might you advocate for equity?  Digital stories are a great way to start group discussion which can be followed up with relevant tools and resources.
